FSS-Oyo Compulsory Submission of Copies of Credentials/Registration Form
FSS-Oyo Compulsory Submission of Copies of Credentials/Registration Form. This is to notify all scholars particularly the 2024/2025 and 2024/2025 ND, HND, PD, PPD, and the PGD GIS students to submit the hard copies of all their Registration Forms to the Admissions and Records Office on or Before 3.00pm, on the16th of August 2024 unfailingly.
Below is the list of all required documents and forms which are to be submitted for all kinds of Students.
- Receipt for payment of acceptance fee.
- The photocopy of acceptance letter from the school portal.
- Copies of forms A-personal details.
- Course registration form (Form B-Blue Form)
- Form C (Medical and accommodation records)
In adding to the above, the following documents must also be submitted.
All ND students should bring both the originals and photocopies of their
- O/Level result(s)
- JAMB admission letter
- JAMB result slip
- Birth certificate
- Local Government Proof of identity letter
- Letter of good manners from their parents, guardian or their sponsor
- And 6 Passport photographs
HND students must bring both the originals and photocopies of
- Their O/Level result(s)
- JAMB admission letter
- The JAMB result slip
- Birth certificate
- Local Government Proof of identity letter
- A Letter of good conduct from their parents, guardian or their sponsor
- And 6 Passport photographs
All Postgraduate (PPD, PD and PGD) students must bring both the originals and photocopies of
- Their O/Level result(s)
- JAMB admission letter
- The JAMB result slip
- Birth certificate
- Local Government Proof of identity letter
- A Letter of good conduct from their parents, guardian or their sponsor
- And 6 Passport photographs
ND II, HND II, PD and PGD as well as all departing students must note that the submission of all documents listed and signing of Matriculation Register is a pre-requisite for the collection of Notification of Results
Lastly Each student should as well submit the photocopy of the TISHIP for record purposes at the Admissions and Records Office.
